To request a replacement for a lost or destroyed certificate for a course completed at Diamond Driving School of Kent, please completely fill out and submit the following form with the information listed below entered in the "Comments" section:
Student's full name,
Dates student was enrolled in course (start - finish),
Student's permit number,
Student's date of birth and gender,
Student's address and phone number - please include a mailing address if different from residence
The fee for a replaceement certificate is $30. We accept payment at the school of cash, checks, or debit or credit cards for this service. Payment can also be made here on our website by clicking on the "Buy Now" button below. Once payment is received (and checks have cleared), the request will be processed. Please allow 14 days for the replacement request to be processed.
